If using your health insurance, one of the partners will need to be the "identified patient" with a mental health diagnosis (e.g. Anxiety, Depression, PTSD). And the relationship therapy is a treatment for that mental health condition or in insurance terminology--couples therapy has to be "medically necessary" to treat the condition.
